This is not the default behavior. It shows first name and last name only once as a default but in your page looks like another plugin trying to edit register form. Do you know which plugin causes this?
If not can you try plugin conflict test? You need to deactivate all plugins and check only with Membership 2 if it solves, try activating plugins one by one.

Yes. Plugin WP-SpamShield is the culprit. Do you know of another anti-spam plugin that does not come in conflict with Membership 2?

I tested Akismet (https://ww.wp.xz.cn/plugins/akismet/) and Antispam Bee (https://ww.wp.xz.cn/plugins/antispam-bee/) for registration form and they are looks like working well. But also you can contact with WP-SpamShield support team, I couldn’t test it because of it’s paid plugin.
Maybe they can provide a solution without the need to change the plugin.
