I have been able to narrow this down a bit. I think it is related to inviation codes. I have installed this on two sites and forgot that I didn’t enter the invitation codes into this site. I believe that is why the submit button is not working, however, that has raised a new issue.
I am not able to enter invitation codes into the raw codes box now. When I click in the box, nothing happens. I am not able to select Auto Generate Codes or Invite Through Email.
Auto Generate Codes or Invite Through Email sections are only available in our PRO version. Please share url for your registration page .

Thank you for your help. Yes, I do have the pro version and because of the private content on the page I am not comfortable putting the link here.
I did find this error in the Error Log:
