Could you clarify where you want to remove these items from? Do you mean on the add/edit order page (when you click on Order and select an order)?
Unfortunately, there is no option to remove all these items from the plugin. You don’t have to fill those in but there is no way to hide them.
Only to show, not remove …

Depending on what you want to do, you would either need to delete the content of certain HTML pages or add a whole lot of CSS to the admin CSS file for your site. Either way, that kind of customization would be beyond the cope of what we’re able to help with here in the support forums unfortunately. They’re meant to help resolve any plugin features that might not be working correctly.
