Hello,
I installed and then removed wp-downloadmanager.

Now I have an extra ‘categories’ section in my Menu area.

Can this be removed? Is there a db table I should delete?

This plugin does not create categories on WP at all. It uses it’s own category so it does not touch the menu or any other core WP stuff. You sure you got the right plugin and not WordPress Download Manager?
