Since some sites use very large numbers of categories and authors, I’m not sure that this option would be very practical, but I’ll think about it. An alternative might be to add category and author exclusion to users and categories pages, or a drop-down selection list. I’ll think about it, and thanks for the suggestion!
true, most sites don’t though. Here’s a suggestion: fire a query at WP DB and if higher than X amount of pages, posts, categories, authors present screens like these (for advanced users); otherwise something user friendlier.

When I thought about alternatives previously, I figured that for a short list it wouldn’t be hard, and for a long one it wouldn’t be practical… or in either case you might be approaching from the other direction (“included” vs “excluded”) or by some other overlapping factor (date, image type, parent category, etc.). In my own usage, date + category type was most useful. At a site with many authors, I might work differently.
