Reply-to: autofill wrong info

My Contact Forms used to work fine. Ever since the latest updates when a customer enters contact information the email that gets sent has 2 emails into the reply-to field. This creates a problem when hitting the reply button as it goes to the original user and my email for some reason even thought It is not being called in the additional headers code. Can someone help me out here on what may have caused this? Thanks.
