We have wordpress setup to “Require Admin Approval” for all (roles) registrations. This has worked in the past but now its automatically approving all registrations automatically without review. We have not changed any UtlimateMember settings or preferences.

I checked my local site and all works well. Try to do a conflict test – https://docs.ultimatemember.com/article/96-how-to-do-a-plugin-theme-conflict-test.
