Required Fields Not Working

I have ALL of the fields marked as required, with all the correct parameters/values set for each required field, yet I can submit my form with only one field filled out. I have all my plugins up to date, database up to date.. I am loading another FormAssembly throughout the site via javascript (I can’t load two forms via javascript, if I do this the forms are stacked and show together on all pages despite using different IDs)

I also had to manually type in the asterisk for all but the last 3 fields.
