Reset Password Email missing URL

    Hello – When our users click on reset password and then enter their user name/email they receive the following email that is missing the URL to click to reset their password. Any help would be appreciated.

    Someone has requested a password reset for the following account: Site Name: <site name> Username: <user> If this was a mistake, just ignore this email and nothing will happen. To reset your password, visit the following address:
