Responsive problems

I have a problem adding a checkbox, everything seems fine on desktop version, but on mobile version, if you click the checkbox text, the optional form opens but the checked checkbox is missing.

also have a problem with the select box, when you select one of the options, 2 fields must appear to fill in, but in the mobile version it does not pay attention, only when you rotate horizontally or in desktop version.
