I see it is a simple form, I imported it to my website and had no issues.
Could you share the form URL and we can try to submit and check the console logs?

Since the form works on our end there’s probably something specific on your site that was causing it, it could have been another plugin or theme so there’s slim chance that @djohnson2021 issue is caused by the same thing.
Please let us know if you can setup a new site where the issue could be replicated so we could investigate it further there.
@djohnson2021 can you please start a separate thread for your issue and share the URL of your form there so we can check this further.
