Search for plugin

Hi.

I’m searching for plugin that has ability to add user custom fields with specified condition: ability to add multiple values displayed as text/select or whatever.
I’m developing ability to create user field Licensed Sites, and want to put in this field values like :
site -> license,
another site -> license
…

Can anyone direct me to this type plugin or help to find it ?
