    I am looking for build a form similar like https://www.fastfixusa.net/fix/ in this page, mainly there is two issue I am facing problem, most important is to setup the select field with radio buttons field (there is different value for each selection) and multi-steps for the next form, it will be best if the plugin providing such kind of functions, I have already got a tutorial to develop a multi step form by the plugin but it will be great to get them at a glance. Anyone please help me to find out the solution.
