“Select Form Template” when saving draft or publishing

New installation of GiveWP, have uninstalled and cleaned database and tried a few different times.
Each time I try to save a Donor Form I get the following:
–Select Form Template
–Please activate a “Form Template” to save the donation form. Select a template —by clicking on the “Form Template” tab and “Activate”.
even when I have already selected the form type. This occurs whether Legacy or Multi-Step have been selected.
The resulting form also doesn’t allow me to input a dollar amount.
