Did you rename the database after configuring the options with Form Maker? Also, it might have become inaccessible for some reason.

The database was not renamed; and it was apparently a valid connection, because the editor retrieved the table name and columns. So, I have submitted a support request form.

Our development team has successfully connected to your database, and added the options. If you check your published form, you will see the options appearing properly.
Did you expect to see them on the form editor? Note, that they only appear on front-end.
Also, we noticed that the entries were duplicated, but they were duplicated on the database itself also.
Please have a look at the select field on your form, and let us know if you notice anything odd.

Wow, never occurred to me that the data wouldn’t appear in the back end. Thank you for persevering on this.
The duplication of data was apparently my mistake. All better now.

In the documentation for this option, the last sentence on the page is “On the back end, the option will display the table field name, whereas for the front end it will display the values which have fulfilled the provided conditions.” Perhaps that could be rendered in BOLD for emphasis. I totally missed it.
