1. go to in your plugin directory.
2 open appointment-calendar-shortcode.php file.
open this link
copy all code and paste it in appointment-calendar-shortcode.php file.
Note: if you all ready make any change that will be loss.

how to add some new field in this plugin.
Where you need extra fields.
after email field i walt to add Reason for visit : Dropdown with few option
add First Appointment : Radio Buttons for Yes/No these two field add in form

this is not possible cause of db problem and also have lots off code customization.
