I believe this may be cause by having any special characters in your checkbox field names/values. Try removing them and only using alphanumeric characters. (ex change “&” to “and”).
This is a know issue and will be patched in a future release.

I removed all ampersands and the “selected value does not exist” message is gone, but I’m still getting the “Please complete required fields” message. The only required field is the email address, which is completed.
Oh no. the selected value message is still there, too.

Make sure you’re removing the “&” from both the Name and Value fields in the form, that should do the trick.

It would be good if we could check the form. Would you please export it and share with us?
To do so, please go to the “Formiantor -> Forms” page and click on a little “gear” icon next to the form in question, then select “Export” option.
Then please either put given export code on pastebin.com or put export file on your Google Drive, Dropbox or similar service.
Once it’s there, post direct link to it in your response below.
This way we’ll be able to import form to a test site and check it and, hopefully, this will help us diagnose the issue.
Note: it’s safe to share the form this way as export only exports form template/settings but does not export any submission data and other information of this type.
