Selecting multiple options

For some reason, although I have ticked the selection to allow multiple answers, it’s not working.

As you can see in “sources of income” and “type of deductions”. Is there any way to fix this? Any help would be greatly appreciated! Thank you for the so far, great plug in!! If the customer approves this and we can get it working, I’m happy to leave a great review!!!
