Thanks for responding for quick. I looked at the documentation, but I now realize that it may be a different question/problem.
In Manage Series -> Add New Series -> Upload an image for the series .. once this is saved it shows in the column and has the Name, Description and Icon entered. But if I go to Edit Series it shows “Current series icon: No icon currently”.
I looked in the database to make sure the plugin created the ‘wp_orgseriesicons’, which it did and the icon show the correct images that were selected as icons.
I setup two series and I have the same problem for both where it shows no icon on the edit series page.
On another website it shows the selected icon on the edit series page. That one I setup a couple of weeks ago to do some testing. But if I try to select an image it doesn’t work at all.
Not sure if this is a bug or something else. I have tested using a default WordPress theme and only this plugin activated on both websites and the issue persists.

I think we can recreate this error. We plan to fix it in an upcoming release:
