Series Icon Not Posting

When I go to add a series icon and select “” it won’t save or close the page. It doesn’t let me add it to the Series. I’ve look at other forums and none of the previous fixes worked for me (changing the table in MySQL, editing the taxonomy.php).

Please help me!

Or can you tell me how to add HTML to the description? I really just want an image to display when you open the series. (http://www.desiio.com/pop/series/30-imperatives/)
