Your “Fields Manager” window looks correct. Click a button that matches a field type you want to add in the “Setup New Field” section and you’ll see the “Add a New Field” window. Configure your new field and click the “Add” button. Your field will appear in the form.

After we disabled all plugins and enable them one by one.
It worked even after all was enabled again.
