If you go to the bottom link you will see 2 form field boxes on that product page that say “Last Name :” and the other says “Country of Origin (if known) :”, that is what I need to accomplished for my new wordpress store, that’s why I was asking if your plugin supplies a shortcode for form fields. I have the same question. I currently have a paypal generated button to make a payment for a class and it allows me a form field for the student name.
