Shortcode outputs rather than returning HTML

The plugin works really well.

One thing I noted was that the shortcode just loads the template directly rather than returning the HTML.

This means the form is always output above any other page content when short codes are processed, rather than within the content of the page.

Would be great if the shortcode returned the HTML so that you could include it within or after other page content.
