We are not familiar with this issue. I just checked from our end and found everything’s working finely. I would request you to make sure that you are using the latest version of the plugin. Current version is v3.5.27
If you still have the problem then you can run a conflict test on your site to find out if any plugin or theme is conflicting or not. To do this,
- Deactivate all other plugins except WP User Frontend
- Activate a default theme like Twenty Twenty One
- The issue should be gone then
- Now reactivate all other plugins one by one and check after activating which plugin the issue reappears
- If you find any such plugins then let me know
- If plugins don’t have conflict then try to reactivate your previous theme
- If the issue reappears then your theme was conflicting with WP User Frontend
- You need to contact that theme’s authors

The problem occurs about wordpress language.
If you use wp in english you can see the forms, if you use another language you cant see the forms.
