What do you mean by “it doesn’t show any table”?
i mean that there aren’t any borders around the fields,
in the print screen that attached it can be seen clearly….
A screenshot is of no use. What might help is a link to a page demonstrating the problem.

Try adding:
.sidebar input,.sidebar textarea {border:1px solid #ccc;}
.sidebar textarea {width:320px;}
to the bottom of style.css.
