I’m trying to use this plug in to create a woocommerce store menu on all my woocommerce related pages (products, cart, shop home, etc.). Here are my settings:

I’ve read the FAQs and looked through the settings and I see no reason why the plugin just… does nothing.

Here is an example page. On the right, you see the default side bar, not the store sidebar I’m trying to use: http://readvitality.com/product-category/issues/

Can you try remove all Pages from the condition group in your first screenshot? Then, click on Add New Group and add those same pages to that group.

The problem is that content in a condition group is treated as associated. This means that currently, your sidebar will be displayed only if the content is:

One of the specific pages AND any product AND contains any product category AND has any shipping class

Moving the pages to another condition group would mean your sidebar should be displayed if the content is:
any product that contains any product category AND has any shipping class
OR
one of the specific pages
