This error seems to have occurred because I had more than one form on the page. Removing the other form resolves it. I wanted one pop-up and one in shortcode. Even when I make a copy of the form I can’t add it without the error occurring.

I understand you created two forms. One to display as a popup and the other as shortcode, and you placed the shortcode directly into a page on your site, right? And when doing that, the Pop-up form doesn’t work as it’s somehow mixing with the shortcode form and returning a “This field is required” message.
Can you please let us know where exactly you are placing the shortcode? Is it on a page, a post, a sidebar?

Correct. They were both on a post.
