Signup Shortcode Plugin Add On

I have not used this yet, but when I watched your video it looks like the form shows up on with web page rather than as a popup like the image implies. How can I make it a popup? Also, how can I add the short code to a button? The reason I ask is that I cannot figure out what the individual page slugs so I can add them to the buttons for the two memberships that I created.
