simple order form needed

Hi,

i need a very simple some-kind-of order form, let me describe:
– there will be a simple table with kids names, age, and gift wishes to Santa, fe:
Peter, 6yrs old, basketball
1. in the last row there would be a santa icon, which means, that if you want to reserve this gift for this kid you would click santa icon, than a form pop ups, where you would enter your name, email and phone number and confirm gift reservation.
2. after confirmation an automatic confirmation email is being sent to an email provided in step 1, and this kid “Peter” becomes inactive, because you already reserved a gift for him
3. website admin, or some admin email receives gift-reservation email

thats all. is there any simple plugin I could use for this purpose? I was looking 2 days for this and nothing, ecommerce plugins are too much gun for this, and contact forms cants “deactivate” gift if it was already reserved before…
