    Everything worked. First time. No issues. No mail problems. No CSS issues. No php conflicts with other plugins.

    Oh and the forms look great.

    I had a functionality question and it was answered by the author within hours.

    There is room for some additional flexibility in layouts and it does not have multi-select fields (if I had to be picky), but I got it do do what I needed so totally happy – 9/10 so giving it 5 stars. It’s better than anything else I’ve seen.
