Simple response counter

I need some advice/suggestions. I’ve spent a fair amount of time trying several poll/counter plugins (YOP, Polls and svl others). None of them allow me to do all of these things:
– Create a question with a simple yes/no or “done” response
– allow change of vote before finishing
– restrict the number of times a visitor can vote (cookie or other control)
– Show numerical tabulation of number of responses before and after clickiing on the response
– give me control for both box/table format and text styling
If anyone has a suggestion I would be happy to receive it.
