Simply not working

Plugin activated, checked ‘on’, checked ‘200’ response, checked ‘custom page’, wrote out custom page, deleted cache, deactivated and reactivated plugins, checked settings again… This plugin used to work, but then stopped working, and I’m not sure what happened… any clue?
