Yes, they are both set to Ajax and set to single optin, but they are on different pages. I created a third, and it works properly as well. I even tried deleting the second form and starting over, but to no avail.

From what I can tell the form on http://ideallymarketing.com/instant-notifications/ is not the form generated via our plugin. When you hit submit it redirects you to a MailChimp optin page, which our plugin should not be doing.
You may want to try removing our shortcode from the <form> element on that page. Ideally, you’ll just want to delete all of the content in the page and start over. You may not be seeing the <form> element, so you might have to switch over to the ‘Text’ tab in the editor.

Perfect! You were right, there was some old code on the page that wasn’t visible from the WordPress editor! I have never encountered that before. I appreciate your help!

I need single optin only.
The form is sending a double optin email even thought I have selected single optin. The form is set to Ajax. Please, can you help me to resolve this issue?
I am using Contact Form 7 and MailChimp opt-in checkbox.
