    I just searched through Google about the tons of emails I’m getting about site lock out notification and it seems lots of people have this issue.

    I just checked out few of my sites and I found something interesting – either this plugin or any “competing” plugins changed the permissions of the .htaccess from 644 to 444 (which means read-only), and since this plugin doesn’t know about this permissions issue, it repeats itself, trying to block IP and sending emails, but since it cannot actually block it due to the permissions, and keeps going in a loop.

    Dear author of this plugin – how about adding a little check about the .htaccess permissions and if it’s read only then “fix” it by changing to 600 or 644?
