@ldekay – I would suggest starting with ensuring all active plugins are up-to-date and since you reported this as a WordPress 4.0.1 installation also update your WordPress as well.
See if these issues persist afterward.

I have this minimal options problem on 7 different sites which are on 5 different server accounts with different companies. Most of the sites are fully up-to-date in core, plugins, and themes (currently having an issue with on server account).

I think that should take care of any questions regarding being up-to-date and an isolated problem.
@ldekay – Actually listing a bunch of sites really doesn’t answer any questions in this case except possibly add more issues and potentially separate ones at that.
I would suggest picking one site you would like to address and trying to sort out the issue on that site (which includes ensuring it is actually up-to-date) and then once that site’s issue(s) has been addressed comparing to any other site for reference and if the other site issues persist then starting again with the next site.
Now, as it is, which specific site are you wanting to reference and what specific settings are you referring to?

I didn’t intend that list to do anything other than give a definitive answer to your question about the site being up to date. I tried to make it 0bvious that the problem does not have anything to do with core or plugins being fully up to date since it occurs both on fully updated and somewhat out of date sites. Also that it’s not an issue to do with the hosting service since it occurs on several different hosts.
I’m content to work on the http://new.mtadamschamber.com/ site to try to resolve this problem, but whenever possible I’ll try to cross-reference any fixes that work against the other non-working sites.
When I use the Gallery Inset button on that site to insert a Basic Slideshow the only display options I have available are:

@ldekay – The first group of settings are the correct ones to see … the recent update removed the “Flash” slideshow due to a security issue and the balance of the settings you are referencing were related to that option.
If you want to take you site to a potentially insecure state (highly NOT recommended) then you can roll back to an older version of NextGEN Gallery but we cannot offer any support for that due to not recommending that action over security concerns.

Thanks for the answer. At least I now understand (and confirmed) the ‘why’ of the problem. Would love to see the soundtrack capability come back, but I can live without it.
