Smart Forms/Smart Donations PayPal Button Broken

We use Smart Forms and Smart Donations together for a donations page. It has been working for years, but recently when clicking the Make a Donation button, it goes to PayPal and we get the error “This organization’s page is broken.”

I don’t see any settings for the PayPal button other than the email address. The account is operating as expected with the email address we set up in the form.

Also, we use a radio button group for preset amounts, it will give a validation error stating the amount must be greater than 0. The only way to get the form to submit is to choose “other” and enter the amount manually.
