Squished text fields

Hello. I’m using the updated version of MailChimp plug-in on my site, http://shelliproffitthowells.com/. I’m using the following html for my header: <p><font size=”5″ color=#0174DF> Sign up for email updates to receive your FREE copy of my eBook.</font></p><img src=”http://shelliproffitthowells.com/wp-content/uploads/2016/02/17-Tips-3D-small.jpg&#8221; />. I’m not using anything for a subheader. The text fields on my site are too narrow to see the text when I enter it. Also, when I tried to do a test sign-up with my gmail account, it didn’t work. I never received a confirmation e-mail, and it didn’t show up on my list. The problem doesn’t seem to be browser specific. I’ve tried it on both Firefox and Chrome.
