Thank you for the topic.
We checked and noticed that, most presumably, the problem comes from the fact that our JavaScript files are not connected to your website.
Let us know whether you are using any plugin that affects the JavaScript or not (e.g. it doesn’t let the JS be connected from the front-end, or performs a sanitization and removes the script tags).
Also, let us know whether you are using any security plugin or not.

I don’t think javascript should be affected with any current implementation. For security purpose, i use wordfence only!

In “Submit button” Event listener i see this: [FunctionLocation]: <unknown>. Is this the root cause?

There is some kind of problem on your website, due to which the files are not connected.
I mean, not that the files have been connected and there is an error preventing the normal operation, but they simply haven’t been connected.
We assume the problem comes from the security plugin you are using.
Please check the logs of the security plugin to see whether there is anything related to the JavaScript or not.
Also, please do troubleshooting to find the conflict.

We recommend you make a DB Backup for your safety (you can use UpdraftPlus).
