Still broken with conditional logic

If the form you are trying to place twice on a page is in AJAX mode AND uses conditional logic, both copies of the form will be hidden. Without this plugin, the first works fine, and the second is hidden.

Nikunj if you are unable to modify the plugin to address this (it may not be a simple fix), I’d suggest adding the warning to the plugin description.
