Are you using a firewall or some sort of proxy service infront of your website? It seems like this could be the issue.
If that is the case we have a setting in the plugin that allows you to configure from which HTTP header should the plugin get the source IP address. You can read more about this in the KB entry WP Activity Log support for Reverse Proxies & Web Application Firewalls.
Can you please check and confirm if this is the issue?
I was not able to get a response from our WP host so did some trial and error.
Selecting the proxy option and the forwarded-for header made the correct IP addresses appear in the log.
