Strange plugin behavior

Good afternoon. Recently, users have started complaining about the strange behavior of the form on my site. Odie write that the form does not send data because it does not see the filled “Date” and “Email” fields. Here is the link to the screenshot https://prnt.sc/gg4pg7
At the same time when I fill out the form, the data is sent without problems. What can cause such an error?
I tried to disable other plugins and changed the template to standard, it did not help
