I hope things are going well for you.
About the form style issue, I reviewed the site’s front end and checked the console; there were no errors. Also, I noticed the site has 3 plugins doing optimization and caching from LiteSpeed Cache, Hummingbird, and the Divi plugin. It could be related to that.
To narrow it down, would you mind performing a few tests?
1. Create a site backup, using the Hosting Panel or any Plugin.
2. Disable the Divi because I tried the following queries with LSCache & Hummingbird Plugin, which aren’t causing the issue.

3. Try changing the form layout from Flat to Default and see how it goes:

Regarding the Email isn’t receiving
Are you receiving the default emails from WordPress, such as plugin updates, password changes, and user account creation? The site is hosted by Hostinger, which offers an email sending feature from its hosting. You can also try sending an email from here: https://prestigebjj.com/wp-login.php?action=lostpassword. If you receive the email, then we need to review the email notification settings of the form.
You can review the email settings from Forminator > Forms > Edit > Email Notifications > Admin Notification> Recipients > Default > Add Admin Mail ID.

Lastly, if neither is helpful, then we require the form export with us.
To share the form export
– Navigate to the “Forminator -> Forms” page in your site’s back-end
– click on a little “gear” icon next to the form in question
– select “Export” option
– copy given export code and paste it at https://pastebin.com
– include link to that paste in your response below.
Note: this export will not include any data that is already submitted – they’ll stay safely on your site and we don’t need them anyway.
