Submission Form Issue

This is a very nice plugin and appears to work fine except for one thing that I just cannot figure out. When I add the submission form shortcode [testimonial-submission-form]the form appears on the page of our site but there is no “Submit” button anywhere. It also is completely removing all of the contents (widgets) from the page’s right sidebar. I’ve contacted the theme developer about this to ask if it was an issue with the theme and was advised that it was not. Just as a test, I added a basic form generated by the Contact Form 7 plugin on the same page to see if it displayed properly with the Submit button and it did so I think we’ve narrowed it down to your plugin. Is there a way to get the submit button in there with the shortcode or a different way? Thanks.
