Otherwise, something had to have changed on the server to cause such a change.
Update the core plugin, possible but hopefully not likely: update to a different plugin that caused some issue with this. Change to the post(s) that removed the shortcode from showing up.
Something is going on that’s preventing the forms from showing.

Still any sort of changes done to the website since it was last working would help us figure out what’s going on a lot more than just me blindly walking through everything.

It wasn’t there when I was logged out, but it was once I logged in. It’s at the end below all the previous submissions that get shown since my user is able to manage submissions.

Are the other two test users at a user level that is allowed to manage submissions/nominations?

What I mean by “below all other previous submissions” is that existing submissions for the task are listed first, and then after all of them are done being displayed, the form to create a new one is displayed.
