I am having a similar issue. The Submissions are in the log in wordpress but Im not getting the emails and my site keeps timing out after submit is pressed.
CristinaC, I don’t think it is simular because I even cann’t submit!

Please try to delete the date field and add again.
Edit Form ▸ Delete Date Field ▸ Update Form ▸ Add new Date Field and Update

Please try the following.
1. Take a look in WP Admin ▸ Settings ▸ General section and make sure that WordPress Address (URL) & Site Address (URL) are the same. Please look through this link too.
2. In /wp-content/plugins/form-maker/js folder please find “frontend” folder, then right click on it. In File permissions check all permissions to be (777);
or
3) In /wp-content/plugins/form-maker/js/ folder please find “frontend” folder and check all .js files to have permission (777);

I have looked at the settings from your point 1. They are equal.
At your point 2 it was 755. I changed it to 777 but no success.
At your point 3: in the folder “frontend” I only find one item .gitignore It has no bites.
So please a next step 🙁
