Submit Button missing

The submit button on this quiz is now missing. It was working when I created this quiz back in May, but has now disappeared. I removed the CSS formatting in case that was causing the issue, but it did not solve the problem. I tested it in Chrome and IE, the button does not show up in either case.

I also tried creating a whole new quiz, thinking maybe there is something going on with the old quiz that isn’t compatible with the updates done to the plugin, but the new quiz I created also had no submit button.

I saw there was another update of this plugin, tried that. Still no submit button.

There used to be pagination in the old version of this plugin which is now gone, and I think it may have something to do with that, but I can’t figure out how to fix it.
