Submit button not working

    I have an issue with using QSM. For some reason, the submit button does not work (it is unclickable).
    I’ve also noticed some errors in Google console:

    Uncaught TypeError: Cannot read property ‘id’ of undefined in
    qsm-quiz.js?ver=7.1.10:1153

    and

    Uncaught TypeError: Cannot read property ‘enable_quick_result_mc’ of undefined in
    qsm-quiz.js?ver=7.1.10:1188

    I’ve disabled all of the plugins with the exception of Akismet Anti-spam and Wordfence but there was no difference. Previously this plugin worked (I haven’t used it in a while) but I don’t know what happened now. I am not sure if I am missing something.
