Do you have a public link where we could see the form in action?
Are you willing to enable the debugging setting we have in our settings area, and submit a test submission to the form or let us submit to it and see if we get added? The debugging would add logging support that may reveal more information about what’s failing.

Issue resolved. I contacted support and they checked and saved the “Bypass Constant Contact cron scheduling” which was the issue. Specifically with regards to the cron job intended to communicate with Constant Contact’s API to add the data. Thanks
