I do not see the form loaded at all so there is not much I can do.
If you used MotoPress to insert the form it might be better to ask them for support.
If they can’t help you, please let me know exactly how and where you inserted the form, maybe include a screenshot as well, so we can take it from there.

Hi @osbornefaith,
MotoPress license expiration does not affect plugin functionality. Try to embed form without using Visual Builder to test it.

@bihar1 Please start a new thread where you describe your issue in more detail and share a link to the page where we can test the form.
@osbornefaith Please share a link to a a normal page (without the visual builder) with the form in it if you have issues there as well.
