Subscription customers charged double item’s value

Recently on an e-commerce site we manage, a rare issue has come up where a customer is charged double the value of their order when their subscription renews. It’s only happened 3 times so far and not always to the same people. One customer had it happen on 2 consecutive renewals, but the most recent order had no issues while another has been a subscriber for more than a year but had it happen to them last week.
If I look at the order in WooCommerce>Orders the quantity and price are correct, but the total and tax are doubled which gets charged to the customer.
I can’t replicate the issue my self, and the vast majority of orders and renewals have had no problems so I’m not really sure how I can deal with this issue, or perhaps its a bug with WooCommerce?
